We want all students to participate to their full capacity no matter their family situation. If you need financial assistance to access any district programs (including sports), please contact your student's counselor. Shoreline School District is committed to providing a learning community which engages all students regardless of financial barriers. All fees must be taken care of prior to our first meet (deadline Wednesday, March 8th). The Athletic fee is $100 and the ASB card is $40. There are 2 fees required for participation in Track and Field. It is required for each athlete to have an OHSAA Physical Form on file BEFORE he/she can participate. The voice of John Lennon, seen here in 1963, will appear on The Beatles new record, says his former bandmate. Students will not be allowed to turn out until all forms have been finalized. The Beatles will release a final record, using John Lennons voice via an AI assist. If you have filled out the forms for another sport, you will simply need to add Track and Field and do the consent forms.
There are requirements for both Parent/Guardian and Students to sign the forms. It takes about 10-15 minutes - and I HIGHLY ENCOURAGE you to fill the forms out with your student. By clicking the link below, you will be prompted to start the process.
It is completely online!! So no more tracking down random forms, kids losing physical copies, or “accidentally" turning it in to a teacher with their homework. This is in place of the “old” athletic packets and forms. You will be required to sign your forms once per year and after any update.We have a new method of doing our paperwork for the District. In addition, you will be able to print the MHSAA health questionnaire or physical examination form directly from your FinalForms site and take that with you to the doctor for your child’s physical (no need to pick one up from school). The most exciting news is that FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving you time. FinalForms allows you to complete and sign athletic participation forms for your students. We are very excited to announce that the Oakridge High/Middle School Athletic Department has partnered with FinalForms, an online forms and data management service.
